CIVHC is pleased to announce the CO Health Change Agent Chat Series, a collection of interviews with boots-on-the-ground organizations and individuals working to advance lower cost and better care for all Coloradans.

Join us for lunch! 12pm-12:30pm on the 4th Friday of every month.

In 2012, the Southeast Health Group (SHG) received a three-year, $1.4 million award from the Centers for Medicare and Medicaid Innovation to develop an integrated care program called TIPPING POINT: Total Integration, Patient Navigation and Provider Training Project.

Tipping Point leaders implemented Health Navigators to identify high-risk patients and work to empower and educate them regarding their health situations. Additionally, they partnered with Otero Junior College to offer a one-year Community Health Worker Certificate and a two-year Associates of Applied Science.
